How Trois Pierre — Soins à domicile collects, uses and protects your personal information, in accordance with Québec's Law 25 on the protection of personal information.
1. Who We Are
This policy describes the practices of Trois Pierre inc., operator of the Trois Pierre — Soins à domicile service, regarding personal information collected through the website, appointment requests and the provision of in-home services.
2. Information We May Collect
Depending on your request and the service required, we may collect only the information necessary, including:
- your identification and contact information, such as your name, email address, phone number, and area of residence;
- your availability and the type of service requested;
- the information necessary to provide the service, such as the visit address, date of birth, medical request, and relevant health information;
- administrative information related to the appointment, billing, or payment, where applicable;
- for the companion care service: general mobility needs, lifestyle habits, home access instructions, and the contact information of a person to reach in case of an emergency;
- certain technical information related to visiting the site, such as IP address, browser type, and date and time of connection, which may be automatically logged by the host.
Important: the current appointment request form does not automatically transmit your information to the site. It prepares an email in your email application, which you must then send yourself. Do not include any sensitive medical information, health insurance number, detailed diagnosis, complete list of medications, and do not attach any prescription or medical document in a regular email.
3. How Information Is Collected
Information may be collected when you communicate with us by email, by phone, in person, through the form that prepares an email, or, once activated, through a secure platform for booking appointments and transmitting documents.
This policy will be updated before the activation of Jane or any other platform that directly collects personal or health information.
4. Why We Use This Information
- to respond to your request and verify whether your area is served;
- to schedule, confirm, and carry out an in-home appointment;
- to verify the medical request and prepare the required equipment;
- to ensure the continuity, safety, and quality of professional services;
- to transmit the sample and necessary information to the designated laboratory;
- to assess a companion care request, organize the visit, and ensure the safety of the person receiving care and of the companion;
- to communicate with a family caregiver, only as authorized;
- to manage billing, payments, access requests, complaints, and legal or professional obligations.
5. Consent and Health Information
Health information is sensitive information. It is collected, used and disclosed only when necessary for the requested service, with your consent or when authorized by law. Where separate or express consent is required, it will be requested from you clearly.
You may withdraw your consent to an optional use or disclosure. Such withdrawal is not retroactive and may limit our ability to provide a service when certain information is necessary for it.
6. Who May Have Access to Your Information
Access is limited to those who need it to perform their duties. Depending on the service requested, certain information may be shared:
- with the laboratory or healthcare professional named on the request;
- with technology, hosting, payment, or appointment management service providers acting on our behalf;
- with an insurer, professional advisor, or competent authority when necessary or required by law.
We do not sell your personal information and we do not use it for advertising purposes without appropriate consent.
7. Processing Outside Québec
Certain technology or hosting providers may process or store information outside Québec. Where applicable, reasonable measures are taken to assess risks and ensure information protection in accordance with applicable requirements.
8. Cookies and External Services
The site does not currently use any Google Analytics tool or advertising cookie installed by Trois Pierre inc. The host may, however, use strictly necessary cookies and retain technical logs for the security and operation of the site.
A Google Maps map is embedded on the homepage. When this map loads, Google may receive certain technical information in accordance with its own privacy practices. You can browse the site without interacting with the map.
The site also uses Google Fonts, loaded directly from Google's servers for character display. This loading may transmit certain technical information (such as your IP address) to Google, according to its own privacy practices.
9. Retention and Destruction
Information is retained only for the period necessary for the purposes for which it was collected, subject to timeframes imposed by applicable laws, professional, tax, or contractual obligations. At the end of this period, it is securely destroyed or anonymized where the law permits.
10. Security Measures
Reasonable administrative, physical, and technological measures are used to protect information against unauthorized access, use, disclosure, loss, or modification. Despite these measures, no transmission over the internet or by regular email can be guaranteed to be completely secure.
11. Your Rights
You may request access to the personal information we hold about you, its correction, its update, or, where the law permits, the withdrawal of your consent and the cessation of certain uses or disclosures. Proof of identity may be requested before processing your request.
